Območje sodobnega Piombina je bilo poseljeno že od antičnih časov. V etruščanski dobi je bilo glavno mesto na tem območju Populonija, zdaj frazione v občini Piombino. Ime Piombino skoraj zagotovo izhaja iz Populina, kar pomeni "Mala Populonija", ki so jo begunci dali majhni vasici, kamor so se zatekli, potem ko so mesto napadli grški pirati (9. stoletje). Prav tako je verjetno, da je bil Piombino ustanovljen že v času dominacije Ostrogota. Leta 1022 je bil na tem območju ustanovljen samostan San Giustiniano, ki je spodbujal dejavnost ribičev, mornarjev in delavcev. Leta 1115 se je Piombino podredil Republiki Pisi in postal njeno drugo glavno pristanišče: oblast je izvajal kapitan ("kapitan"). Med konflikti med Pisani in Genovežani (12.-13. stoletje) je bilo mesto večkrat opustošeno. Leta 1248 je kapitan Ugolino Arsopachi zgradil kanale.
